HHM Health provides care to everyone regardless of their insurance coverage or ability to pay.

To become a patient at HHM Health, please visit our enrollment office on the third floor of our Vickery location at 5750 Pineland Drive.

HHM Health accepts Medicare, Medicaid, Amerigroup, Humana, Baylor Scott and White, Aetna-Medicaid-Better Health, Aetna PCB (must be an HHM Health Provider), Bright Health, Parkland, Molina, Superior, Texas Health Women, United Health Care, Blue Cross Blue Shield, Well Med, Cigna, and Tri Care – Humans. 

After 5pm, your call will be directed to an on-call scheduler or provider who can assist you if possible. If not, they will relay your message to your provider.

If you do not have insurance, our enrollment office can help you apply for Medicaid or help you qualify for our sliding-scale fee.

Notify your clinic immediately so that our front desk team can update your account accurately. Be sure to bring a copy of your new Insurance Card so that our Billing Team can process and submit claims on your behalf to avoid patient financial responsibility.

If you are a patient of HHM Health Pharmacy, or would like to become one, you can call 972-354-8706 and the pharmacy team will refill your prescriptions or reach out to your provider for new prescriptions, if needed. If you call outside of business hours, feel free to leave a voicemail with your information, and we will text your phone number with an update as soon as we can.

Yes, this is especially vital for claims submission, referrals and prior authorizations. If we are not selected as your PCP (with exception to Medicaid) patients risk their claims/pre-authorization being denied. This also is important for internal reporting purposes.  

If you know you will not be able to make your appointment anymore, please call ahead to reschedule or cancel your appointment. HHM Health wants to make sure all patients can be seen in a timely manner, so please be sure to give us notice of your expected absence so that we can accommodate other patients.

Yes, HHM Health has several free interpreters who Speak Spanish, Burmese, Rohingya, Karen, Malay, and Hakha. We also have a language line at all our clinics which is used for our patients to communicate health needs in their respective language.

Yes, the HHM Health Pharmacy provides free delivery. If you would like to make the switch to the HHM Health Pharmacy, please talk to your provider.

Yes, your conversations with our staff are kept private.

For initial newborn visits, please bring your discharge documents. If your child recently visited an emergency room, please bring the ER discharge documents. If your child recently moved into the area or changed medical centers, please bring all vaccination records and medical records.

HHM Health partners with MediCopy to provide patient medical records. To request your medical records, please follow these instructions:

  1. Sign an authorization form at HHM Health or online at www/medicopy.net/patients
  2. Please provide an email address, if possible. Your email address will expedite the process and delivery method.
  3. After your authorization is received, MediCopy will fulfill your medical request and send to the requesting party

A sports physical is given to clear a person’s ability to participate in sports. It does not include lab work, diagnostic testing, or screenings for diabetes, cholesterol, hypertension, or sexually transmitted diseases. This exam is more simplified than an annual physical exam which is a comprehensive exam that aims to follow up on current diagnoses identify new diagnoses and develop a treatment plan.

Day Of:

  • Bathe as you normally do
  • Don’t apply antiperspirants, powders, lotions, creams, or perfumes
  • Bring previous images and results from last mammogram if you have them
  • Eat and drink as you normally would
  • Take any medications as you normally would
  • Consider taking pain medication (over-the-counter) about one hour before your appointment time
  • Wear a two-piece outfit
  • Wear comfortable shoes
  • Leave neck jewelry and long earrings at home
  • Share important information with the mammogram technician during your appointment such as:
    • If you have breast implants
    • If you have trouble standing or holding still
    • If you are breat feeding
    • If you think you may be pregnant
